PRE-SCREENING

A FREE 30 MINUTE CONSULTATION IS *REQUIRED* FOR ALL PERMANENT MAKEUP APPOINTMENTS TO DETERMINE IF YOU ARE A GOOD CANDIDATE FOR PMU. THIS CAN BE
DONE IN PERSON OR VIA VOICE OR VIDEO CALL (IF A CALL IS PREFERRED PLEASE INDICATE THIS WHEN BOOKING).

A CHARGE OF $25 WILL BE INCURRED IF NO SHOWED.

CONTRAINDICATIONS

WE CANNOT ACCEPT YOU AS A CLIENT IF YOU:

~ARE UNDER THE AGE OF 18

~ARE CURRENTLY PREGNANT OR BREASTFEEDING

~HAVE RECIEVED BOTOX OR FILLERS IN AREA TO BE TREATED WITHIN THE LAST 3 WEEKS

~ARE CURRENTLY ON ACCUTANE OR HAVE TAKEN ACCUTANE IN THE PAST 12 MONTHS

~ARE AFFECTED BY ANY AUTOIMMUNE/IMMUNOCOMPROMISED DISORDERS

~ARE TAKING BLOOD THINNING MEDICATIONS

~HAVE TRANSMITTAL BLOOD CONDITIONS LIKE HIV OR HEPATITIS

~HAVE UNCONTROLLED HIGH BLOOD PRESSURE

~HAVE UNCONTROLLED DIABETES

~CERTAIN HEART CONDITIONS MAY REQUIRE DR. CLEARANCE AND ANTIBIOTICS

ANNUAL TOUCH UP

COLOR REFRESH APPOINTMENTS WITHIN 24 MONTHS OF PREVIOUS SERVICE WILL BE BOOKED AT THE DISCRETION OF MY COSMETIQUE PRO.

PREVIOUS PERMANENT MAKEUP

ANY CLIENT WITH PREVIOUS PERMANENT MAKEUP PERFORMED OUTSIDE OF MY COSMETIQUE PRO WILL NEED TO BE PRE-APPROVED PRIOR TO BOOKING SERVICES. THESE MAY REQUIRE REMOVAL OR COMPLETE REBUILDING OF SHAPE/PIGMENT AND ARE CHARGED AS A NEW FIRST TIME CLIENT NOT AN ANNUAL TOUCH UP.

DEPOSIT

ALL APPOINTMENTS, WITH THE EXCEPTION OF FOLLOW UP APPOINTMENTS FOR NEW CLIENT SERVICES, REQUIRE A $100 DEPOSIT AT TIME OF BOOKING TO SECURE THE APPOINTMENT DATE. THIS DEPOSIT WILL BE APPLIED TOWARDS THE COST OF YOUR FIRST SERVICE OR REFRESH APPOINTMENT. DEPOSITS ARE NON-REFUNDABLE AND NON-TRANSFERABLE.

 

ANY RESCHEDULING/NO SHOW WILL RESULT IN FORFEITURE OF DEPOSIT.

CANCELLATION POLICY

*PERMANENT MAKEUP APPOINTMENTS TO BE CANCELLED OR RESCHEDULED, MUST BE  COMMUNICATED TO MY COSMETIQUE PRO NO LATER THAN 2 WEEKS PRIOR TO YOUR APPOINTMENT DATE. FAILURE TO NOTIFY US WITHIN THIS POLICY PERIOD WILL RESULT IN FORFEITING YOUR
DEPOSIT. AN ADDITIONAL $100 DEPOSIT WILL BE REQUIRED TO SET A RESCHEDULED APPOINTMENT DATE. A NO SHOW FOR YOUR APPOINTMENT WILL RESULT IN LOSS OF DEPOSIT AND 75% NO SHOW FEE APPLIED.

 

*ALL OTHER SERCIVES PLEASE PROVIDE A MINIMUM OF 24 HOURS NOTICE TO CANCEL/RESCHEDULE. LESS THAN 24 HOUR NOTICE WILL RESULT IN 50% LATE CANCELATION FEE. IF ANY OTHER SERVICE IS IN NO SHOW STATUS A 75% FEE OF SERVICE NO SHOW’D IS APPLIED AND REQUIRED TO REBOOK.

 

WE RESERVE THE RIGHT TO DECLINE FUTURE SERVICES DUE TO HABITUAL CANCELLATIONS/NO SHOW’S

NO CALL/NO SHOW

CLIENTS WHO DO NOT ARRIVE TO THEIR APPOINTMENT AND FAIL TO NOTIFY MY COSMETIQUE PRO WILL BE DENIED OF ALL FUTURE SERVICES AT OUR DISCRETION.


DEPOSIT WILL NOT BE REFUNDED AND 75% OF TREATMENT COST WILL BE CHARGED TO CARD ON FILE.

LATE POLICY

MY COSMETIQUE PRO ALLOWS A 15 MINUTE GRACE PERIOD TO EACH APPOINTMENT. ANY CLIENT ARRIVING MORE THAN 15 MINUTES LATE TO THEIR APPOINTMENT, REGARDLESS OF NOTIFICATION, WILL RESULT IN A CANCELLED APPOINTMENT. 

 

AN ADDITIONAL $100 DEPOSIT WILL BE REQUIRED TO RESCHEDULE PERMANENT MAKEUP APPOINTMENTS.

PRICING

PRICING MAY CHANGE AT ANY TIME, AT THE DISCRETION OF MY COSMETIQUE PRO.

 

PRICE AT TIME OF BOOKING WILL BE HONORED IN THE EVENT A PRICE INCREASE TAKES PLACE PRIOR TO YOUR APPOINTMENT DATE.

NO GUEST POLICY

GUESTS, INCLUDING CHILDREN, ARE NOT PERMITTED INSIDE THE STUDIO.

RESULTS NOT GUARANTEED

MY COSMETIQUE PRO WILL PROVIDE DETAILED PREPARATION AND AFTERCARE INSTRUCTIONS.
HOWEVER, WE ARE UNABLE TO GUARANTEE RESULTS, AS EACH CLIENTS LIFESTYLE/MEDICATIONS/HEALTH/SKIN 
CARE REGIMN/ETC. ARE DIFFERENT AND MAY HEAL
DIFFERENTLY. IF YOU FEEL YOU MAY NEED AN ADDITIONAL SESSION, PLEASE CONTACT MY COSMETIQUE PRO TO ARRANGE AN APPOINTMENT.

COVID-19

As Permanent Makeup Artists, we are faced with exposure to bloodborne pathogens. We have always cleaned with medical grade disinfectant and have worn the proper PPE. Extra safety precautions and cleaning measures will be taken by My Cosmetique Pro due to COVID-19.

If you were sick, around someone sick, or feel you were exposed to the virus within 14 days of your appointment, please reschedule your appointment to accomodate for a 14 day quarantine period. If you are feeling sick near the time of appointment or day of including runny nose, sore throat, fever, cough, vomiting/diarrhea, or any flu like symptoms, please cancel and reschedule.
